In order to release authorization for a third party to pickup your transcript at the Athens campus, please complete the following form and submit proper documentation.

I authorize Trinity Valley Community College to release copies of my official transcript to the third party that I have designated below. I also attest to the understanding that:
- The designated third party must provide their own valid state photo ID before these documents will be released.
- The designated party will not be permitted to view my transcripts before they are sealed officially.
- The release form is valid for single use by single third party recipient only.
- Any holds that block transcript release will prevent processing of this form; third parties will not be given details on any preventative holds on the student account.
- This form will not be processed if there are any discrepancies in the information I provide and the information provided by the third party at time of pickup.
